Report a Record Issue

This form is for issues with the public record itself.

Use it to report a possible missed requirement, an entry that looks incorrect or inconsistent, evidence that is missing or will not load, suspected misuse of the project’s public material, or a question for the Accountability Partner.

A report is evidence for review. It is not a verdict. The Accountability Partner checks each report against the record and the written rules; Micheal does not determine whether a report about his own compliance is valid.

Nothing is published automatically. Threats, harassment, or private information about anyone will be discarded and are not part of the record. Ordinary request logs are retained by the hosting provider.

What happens to a report

The Accountability Partner reads every report. A possible missed requirement or record error is checked against the evidence and the written rules. If substantiated, the outcome appears in Updates or the Violation Log, with the correction noted. Unsubstantiated reports are closed without publication.

What this form is not for

General comments, encouragement, or messages to Micheal. The record is administered by the Accountability Partner; questions about compliance belong here rather than in a conversation with the participant.
